Why is the ventrogluteal the most preferred IM site?
The site is deep and away from major nerves and blood vessels.
How to landmark the ventrogluteal IM site.
- patient supine or lateral.
- use opposite hand to injection side.
- heel of hand on greater trochanter.
- place thumb on groin.
- point index finger to anterior superior iliac spine.
- extend middle finger back toward buttock to create V shape.
- Inject into the middle of the V shape.
How to landmark Vastus Lateralis.
- hand width above the knee.
- hand width below the greater trochanter.
- use the middle third for injection.
How to landmark Deltoid.
- have patient relax arm by flexing elbow.
- palpate acromion process.
- apply three fingers horizontally with the top finger along the acromion process.
- injection site is three fingers down.
Nursing interventions to reduce IM injection pain.
- pressure for 10 seconds
- apply ice or numbing cream
- choose appropriate needle length/gauge
- sharp beveled needle
- 90 degree angle
- relaxed muscle
- 10 seconds per 1mL
- Distraction
