DDX: Acute Mitral REgurgitation
Papillary muscle or chordae tendineae rupture from MI
Valve leaflet perforation from infective endocarditis
DDx: Chronic Mitral Regurgitation
Fibroelastic deficiency syndrome - MCC
Mitral Valve Prolapse
Symptoms: Acute MR
Severe dyspnea, tachycardia and pulmonary edema
Physical Exam: Acute MR
Harch apical systolic murmur
Symptoms: Chronic MR
Exertional dyspnea
+/- palpitations or atrial fibrillation
Physical Exam: Chronic MR
High-pitched apical holosystolic murmur radiating into S2
May have an S3
ECG Findings
May show ischemis in the inferior or anterior walls
LAE and LVH
Management: Acute MR
Emergency Surgery
Start with 02 and PPV
Nitrates to reduce afterload
Dobutamine if needed
