Acute Inflammation Unclosed Epiphyseal plates (children) Only a low dose over a fracture Over breast implants Reduced sensation or circulation

why would you use non thermal US?
to accelerate healing through altering the membrane permeability in the tissue
Contraindications for Cold
Cold hypersensitivity or intolerance
Cryobulinemia and paroxysmal cold hemoglobinuria
Cold allergy/raynaud’s disease
Over regenerating peripheral nerves
Impaired circulation, sensation, or mentation
Precautions for Cold
Over a superficial main branch of a nerve Over an open wound HTN Poor sensation or mentation Very young or old
indications for cold
Acute/Subacute swelling Inflammation Pain Spasticity Management Post Exercise or stretching Mild Burns
Physiologic effects of cold
Decrease local temperature followed by hunting response
Decrease circulation and vasoconstriction followed by increased circulation and vasodilation
Decrease edema
Decrease muscle excitability, muscle spindle depolarization
Decrease local metabolism
Decrease sensation
stages of cold
Intense Cold (within 3 minutes)
Burning or Aching (4-7 minutes)
Local Analgesia (8-15 minutes)
Numbness (15-30 minutes)
Contraindications for Heat
Acute inflammatory conditions; Acute spasm Thrombophlebitis Impaired sensation, mentation Malignance IR over eyes Recent or potential hemorrhage
Precautions for Heat
Over an open wound Pregnancy Impaired circulation Poor thermal regulation Edema Cardiac insufficiency Metal in the area Over areas where topical creams were recently applied Demyelinated nerves
Indications for Heat
Subacute or chronic phase of healing
Decreased ROM (ie prior to stretching)
Muscle guarding or spasm
