Concerning labs
HR<60, >100, O2 <94%, increased WBC, hypo/hyperkalemia, positive pregnancy test, increased PT/INR/aPTT
airway, breathing, cardiovascular status, mental status, incision site, temp, IV fluids, other tubes
Perioperative nursing interventions
Respiratory interventions
O2 95% or higher, assess lungs every 4 hrs., positioning, oxygen therapy, breathing exercises, movement
Cardiovascular interventions
Report 15-20 BP difference, assess for pulse deficit, decreased pulse could be cardiac depression, pulse pressure could indicate fluid imbalance, abnormal heart sounds could be shock/hemorrhage/ or effects of drugs
Neurologic interventions
assess awareness and orientation by calling name/touch/giving simple commands, assess LOC
Neuromuscular interventions
Lightly prick skin w/monofilament and ask if they feel sharp or dull, ask them to move each extremity assess strength of each limb
GI interventions
assess for nausea continuously, put in side-lying position, adequate hydration, promote mobility,
Urinary interventions
catheter for urinary retention, output should be close to total intake for 24hr period, report output of less than 30mL/hr. or 240 in 8hr shift
Integumentary interventions
assess tissue integrity at least every 8hrs, notify surgeon of serosanguinous drainage lasting more than 5 days, proper nutrition
