Dx of atopic
Itchy skin plus 3 or more of:
Visible flexural dermatitis (cheeks / extensor surfaces if < 18 months)
Personal hx of flexural dermatitis
Personal hx of dry skin in last 12 months
Personal hx of atopy / 1st degree relative if < 4
Onset < 2
Subtypes
Endogenous - atopic, varicose, discoid, seborrhoiec
Exogenous - contact irritant, contact allergic, photodermatitis
Tx of infected eczema
Topical Fucidin H (every 12 hours) - mild steroid
Topical Synylar C (every 12 hours) - potent steroid
Eczema Herpeticum
Areas of rapidly worsening, painful eczema
Clustered blisters
Punched out erosions (1-3mm, uniform in appearance)
Possible fever, lethargy or distress
Complications - hepatitis, pneumonitis and encephalitis
Topical calcineurin inhibitors
Tacrolimus and pimecrolimus
Alternative to topical steroids
Suppress T lymphocyte responses
