Dx criteria of schizophrenia
Duration 6 months
Social or occupational deterioration
2 or more in the past 1 month: delusions, hallucinations, disorganized speech, disorganized or catatonic behavior, negative symptoms
Negative symptoms of schizophrenia
Anhedonia, flat affect, alogia (poverty of speech), avolition, poor attention
Metabolic basis of schizophrenia
Increased activity of dopaminergic pathways. Drugs typically dopamine receptor antagonists
Worse prognostic factors in schizophrenia
Early onset, poor social support, negative symptoms, family hx, gradual onset, male, poor premorbid functioning
Typical neuroleptics
Low potency - Chlorpromazine, thioridazine
High potency - trifluoperazine, haloperidol
D2 antagonists
Atypical neuroleptics
Risperidone, clozapine, olanzapine, quetiapine, aripipazole, ziprosadone
5-HT2 antagonist, D2 antagonist
Neuroleptic malignant syndrome
Confusion, high fever, elevated BP, tachycardia, lead pipe rigidity, sweating, increased CPK
Dx of schizophreniform disorder
Symptoms between 1-6 months
Dx of schizoaffective disorder
Meet criteria for MDD, manic episode, or mixed episode
Have delusions or hallucinations for at least 2 weeks without mood disorder symptoms
Have mood symptoms present for a majority of psychiatric illness
Brief psychotic disorder
Lasts between 1 day to 1 month
Delusion disorder
Fixed delusions for at least 1 month without major dysfunction in life
Koro
Pt believes penis is shrinking and will disappear
Asia
Amok
Sudden outbursts of violence for which the person has no recollection, often with suicide.
