Rabies virus?

Rhabdovirus
Rna virus
Infect cns or salivary glands of mammals like cats, foxes,dogs, cayoytes,
Convey virus through saliva
Incubation period 3-7weeks
Virus traveles through nerve to brain from biting site
Form negri bodies site of viral replication and transcription

Clinical features?

A

Fever, paresthesia of biting site
Panic attacks
Hydrophobia: involuntary violent contractions of diaphragmatic muscles, acessory muscles and causes spasm of laryngeal muscles by drinking fluids
Air currents: skin become sensitive to temp
Cns: after 10 days encephaliticform delusions, hallucinations, spiiting and mania usually fatal
Hyperpyrexia,

Investigations?

A

Observe animal usually die after 5 days if rabid animal

Pcr or csf for definitve diagnosis

Treatment?

A

Wound clean with water and antiseptic solution
Wound should not clean
If ivg given then infiltrated locally also and remainder IM
Symptomatic treatment: no treatment give iv diazepam 10mg plus chlorpromazine

Post exposure prophylaxis?

A

Wound should clean
Ivig: 2m/kg or 40units per kg
Vaccine: 1ml IM 0,3,7,14,28
If vaccine is not there then wait for symptoms and observe biting animal

A

Diploid cell vaccine IM 1ml on0,7,21,28 for high risk persons
