A

<15 years old: Exhibiting any of the following signs of inadequate perfusion:

  • AMS (Decreased LOC, confusion, agitation)
  • Tachycardia (<5y/o: ≥180 or ≥5y/o: ≥160)
  • Cyanosis/Mottling/Pallor
  • Diaphoresis
  • Difference in Central vs Peripheral pulses
  • Delayed capillary refill
  • Systolic BP <70+(2xAge)

Adult Shock Definition

A

“Shock” is defined by the following: (Age ≥15 years):

  • Systolic BP <80 mmHg OR
  • Systolic BP <90 mmHg AND exhibiting any of the following signs of inadequate perfusion:
    • Altered Mental Status (Decreased LOC, Confusion, Agitation)
    • Tachycardia
    • Pallor
    • Diaphoresis

Burn Center Criteria (Adult)

A
  • ≥5% Full Thickness
  • ≥20% Partial Thickness
  • Suspected Respiratory involvement or significant smoke inhalation in a confined space
  • Burns to sensitive areas such as Hands, Face, Feet, Genitalia or Circumferential
  • Electrical burn w/ ≥120 volts

A

An ALTE/BRUE is defined as an episode involving an Infant that is frightening to the observer and is characterized by one or more of the following

  • Apnea (Central or Obstructive)
  • Color Change ( Cyanosis, Pallor, Erythema)
  • Marked changed in muscle tone
  • Unexplained chocking or gagging

Transport for ALTE/BRUE

A
  • ALS if patient is symptomatic
  • BLS if patient is asymptomatic
  • Private transport is acceptable for asymptomatic patient IF:
    • Transportation is available now
    • The parents/caretaker are reliable
    • The parents/caretaker understand the importance of evaluation

A
  • Who activated 911 & reason why
  • All circumstances pertaining to consent issues during a patient encounter
  • The presence or absence of any impairment of the patient/DDM, such as by drug/alcohol
  • The ability of the patient to comprehend & demonstrate an understanding of their illness/injury
  • The patient/DDM has had the potential risks & potential outcome of non-treatment or non-transport explained by EMS such that the patient/DDM can understand
  • The reasons for the AMA, the alternative plan if any & the presence of any on scene support system such as family, friend, neighbor.
