Annually (the month the corresponds with the station number unless it is greater than 12 then go by the last digit of the station number)
Blood work must be done 2 weeks prior. And if you are under 40 then you must have a PFT come give you the gurkin test prior to your physical.

Paperwork picked up at fleet services. After testing return paperwork to fleet services. Bring copy of emissions to Fire Maintenance.
