How does joint swelling manifest?
Redness, swelling, morning stiffness for at least 30 minutes
Diseases that are Monoarticular
Gout, trauma, septic arthritis, lyme disease, *osteoarthritis
Diseases that are Oligoarticular (2-4 joints):
Reactive arthritis & psoriatic arthritis
Diseases that are polyarticular (5 or more):
Rheumatoid Arthritis, Lupus (SLE)
Only 2 diseases that affect the DIP:
Osteoarthritis, Psoriatic Arthritis
Diseases that affect the first MTP (big toe)
Gout, Osteoarthritis
What 2 joints will be spared in individuals with Psoriatic Arthritis and Osteoarthritis? / Which diseases will not spare these?
Wrist, MCP / Rheumatoid Arthritis, Lupus, Pseudogout
Neuropathy suggests:
Vasculitis
What are contraindications to aspirating a joint? / When do you use a small needle? (22 F)
Never pass through cellulitis or psoriatic plaque b/c of risk of introducing infections / When patient is on Warfarin
WBC count for: Noninflammatory / Inflammatory / Purulent
Under 2,000 / 2,000 - 75,000 / over 100,000
What you use to to look at crystals / type of gout crystal & characteristic / pseudogout type of crystal & characteristic
Dark field microscopy / Monosodium urate, negatively birefringent / Calcium pyrophosphate, positive birefringent.
