Sepsis Flashcards
SIRS Criteria
Systemic Inflammatory Response Syndrome
- Temperature Above 100.4 or Below 96
- HR Tachycardia >90bpm
- Tachypnea >22breaths per min
- WBCs >12000 or <3000
Sepsis
Infection Overwhelms the Bodies Defenses
Sepsis S/S
*Altered Mental Status
*Fever, WBC >12000
*Tachycardia >90bpm
*Tachypnea >2bpm
*Hypotension
Decreased Cap. refill-Mottling
Mean Arterial Pressure
> 65
Early Signs Septic Shock- COMPENSATING
- Warm/Flushed Skin
- Decreased BP, Increase HR, RR, Fever
- Anxiety, Restlessness
Late Signs Septic Shock- UNCOMPENSATED
- Vasoconstriction
- Cold/Clammy, Decreased Cardia Output
- RF-Oliguria
- Coma, Severe Hypotension, Hypothermia
1st Intervention for Septic Shock
Increase Perfusion- Fluid replacement (NS) 30ml/kg
2nd Intervention if 1st doesn’t work
Vasopressors (Norepinephrine)
Vasopressors (action)
Increase Systemic Vascular Resistance, BP, MAP, CVP,
Norepinephrine Interventions
- Admin. Central Venous Line
2. Monitor for Dysrhythmias.
What can be given if BP still low after fluids/vasopressors given?
Inotropic-Dobutamine (CVL admin) Increases heart contraction
Vasoactive Drugs
Vasopressors, Vasodilators
Diagnostic Tests
CBC, Lactate, Glucose
Central Venous Pressure
8-12mmHG- RIGHT SIDE OF HEART
Others Drugs Used in Septic Shock
Steroids (Hydrocortisone)
Broad Spec. Antibiotics
PPI (Omeprazole)
VTE Prophylaxis (Heparin, Enoxaparin)
