Uses of ECT:

A

Usually second line Treatment for depression.
Used for people who can’t tolerate antidepressants
Urgency (risk suicide or death)
Also mania, schizophreniform, puerperal psychoses, catatonia, NMS, epilepsy

What are the NICE guidelines surrounding ECT:

A

Only for rapid and short term Sx improvement, if other Rx failed, or life threatening situation, severe depressive episode, catatonia, prolonged or severe manic episodes
Not as maintenance therapy

Procedure of ECT:

A
Brief square wave pulses, unilateral (less cognitive impairment, less effective)
Seizure threshold (17-800mQ), Stimulus dosing (by titration), Short General anaesthetic + muscle relaxant, seizure monitored (EEG) x2 per week. 6-12 treatments.

Side effects of ECT:

A

Retrograde (before) and anterograde (after) amnesias.
Status epilepticus, laryngospasm, peripheral nerve palsy. caution if at risk of CVA/ have raised ICP (intracranial pressure

A

50% will have at least one further episode.

Those who have had more than 2 episodes will have a 70-80% likelihood of further recurrence
