Name the muscle and the nerve/vein/artery that supports it:

Trapezius!

Innervated by the Accessory Nerve - Cranial Nerve XI 

The TRANSVERSE CERVICAL arteries and veins provide vasculature

How would you identify an individual with damage to the accessory nerve?

If it is unilateral, they would have a droopy shoulder or weakness in one side. Bilateral would have problems like that bilaterally.
