  • marked swelling and pain over patella
  • point tenderness
  • reduction in extension strength

Laterally due to imbalance between lateral pull and mechanisms resisting this:

  • medial more horizontal pull of vastus medialis
  • more anterior projection of lateral femoral condyle - deeper slope for larger patellar facet
