A
  • Severe blunt trauma
  • Penetrating trauma to the chest or neck (>1/2 due to MVA)

A
  • Respiratory distress
  • Hemoptysis, cough
  • Subcutaneous emphysema
  • Wound opens into pleural space –> large pneumothorax with persistent air leak
  • Pneumomediastinum on Chest X-ray

What is the treatment of TBTI?

A
  • May need endotracheal intubation
    -With bronchoscope
  • Multiple chest tube insertion required
    • Continues to bubble vigorously with little resolution of pneumothorax
  • Surgical repair: Thoracotomy
