Light’s Criteria
Pleural fluid protein > .5 times serum protein
Pleural fluid LDH .67 times serum LDH
Pleural LDH >.6 times upper limit of normal serum LDH
This means exudative.
chylothorax
pleural fluid triglycerides >110 mg/dl with chylomicrons
NSIP
Nonspecific Interstitial Pneumonia is the most common interstitial lung disease secondary to connective tissue disorders.
Treatment of NSIP
Stop offending agents and start systemic corticosteroids
20 mg max in scleroderma due to renal crisis.
60 mg max in other aptients
1mg/kg/day
Also cyclophosphomide.
Contraindications to early enteral nutrition
Severe ileus bowel obstruction bowel ischemia intractable vomiting and diarrhea high output fistula severe hemodynamic instability.
Symptoms in COPD exacerbation
Dyspnea
increased cough
change in sputum production
Severe COPD
FEV1/FVC less than 70
FEV1 30-50% normal
RF for spontaneous pneumothorax
tobacco abuse Marfan homocystinuria thoracic endometriosis family history
Treatment of mild, moderate and sever pneumothorax
Mild 2-3cm air obs and oxygen
Severe >3cm needle aspiration and chest tube insertion if still symptomatic.
