what is personalisation?
knowing that every individual has their own strengths, preferences and wishes that need to be put at the centre of the process so they have choice and control
what is a support plan?
a document where preferences and day to day requirements for care and support are acknowledged so they can live with dignity and respect in the community
what are the features of personalisation?
choice and control, self assessment of needs, personal budgets, co- production, changing roles of professional
what is a self assessment of needs?
local authority deciding if long term support is needed, the assessment will see what the users wants are, the support they have and what they need, they will see if eligible and decide on personal budget
what is a home adaptation?
changes to an individual’s home to make it safer and more comfortable to live independently
what is a personal budget?
an agreed amount of money used to carry out aspects of provision to see out their support plan (means tested cash payment depending on financial situation)
what can they spend personal budget on?
anything as long as it’s in their care plan, they can employ personal assistant to help them
what is direct payment?
payment to individual/ nominated person, gives responsibility to make decisions and can spend on anything related to care plan, gives more choice and control
what is a managed account?
not paid directly to person, managed by local authority, individual must know amount of money so they can have some choice
what are key features of personalisation?
changing role of the professional, co-production, personal budgets, choice and control, self assessment of needs
what are some benefits to personalisation?
• user obtains and keeps control
• user can remain in own home
• inclusion in community
• improved information
• improved quality of life
what is a positive impact of direct payments?
• quick access to services
• individual can have support when needed
• no need for individual to wait for local authority to approve payment
• money to pay things immediately
what are the positive impacts of staying in own home?
• remain in own home (familiar and have a sense of belonging)
• feel comfortable and safe
• know home area
• know people around them
what are positive impacts of new opportunities?
• support is given when they want/need it
what are positive impacts to information and guidance?
• allows better choices
• users more confident making choices
