Acute uncomplicated pyelonephritis treatment

A

FQ resistance 7 day course of cipro or levo
Bactrim 2nd line if FQ resistance >10%->14 day course
3rd gen cephalosporin (Rocephin,ceftriaxone) 1g IV or cefotaxime 2g IV q4-6h, followed by 10-14days of oral ceph

Cystitis treatment

A

Bactrim 1st line if resistance20% for Bactrim. Avoid if SCr<60. BIDx5days
Fosfomycin (monurol) 3rd line. Broad spectrum. Pregnancy categoryB. 3 gram PO once
B-lactamas (Augmentin, Omnicef, Vantin, cedax) lesser likely to be effective
FQ(cipro, levo) broad spectrum activity against p aeruginosa. Avoid

Bactrim/Septra DS 10days
FQ 7 days
Beta lactamas 14 days
IV/IM ceftriaxone. 7

Prostatitis treatment

A

FQ (cipro and levo) they penetrate prostate and kidney tissues well
14 days for acute prostatitis
Beta Lactam will work if IV to reach desired tissue concentration
