Ways of Treating Peri-Implant Mucositis & Peri-Implantitis Flashcards
Introduction
What is Peri-implant mucositis vs Peri-implantitis?
- Dental implants are popular for replacing missing teeth due to high success rates and durability.
- Peri-implant mucositis is an inflammation of soft tissues surrounding the implant.
- Peri-implantitis involves inflammation and loss of bone around the implant .
- If left untreated, peri-implantitis can lead to implant failure.
- This presentation will discuss the ways to treat both peri-implant mucositis and peri-implantitis
Etiology
Peri-implant mucositis:
Main cause →biofilm accumulation → results in inflammatory lesion
Main bacteria :
- F. nucleatum
- P. intermedia
- Eubacterium
Etiology
Peri-implantitis:
Main cause → bacteria build up → poly-microbial anaerobic infection
Main bacteria :
- Staphylococcus aureus
- Fusobacterium
- P. gingivalis
Prevalence
- peri-implant mucositis: ~ 43.5%
- Peri-implantitis: ~ 21.9% .
Prevalence rate can vary widely depending on several factors including patient demographics , implant design , patient-related factors , and length of follow-up .
- Maintaining good oral hygiene practices and attending regular follow-up appointments with a dental care provider can help prevent and manage peri-implant diseases .
Treatment of Peri-implant Mucositis
Treatment of Peri-implantitis
Non-Surgical Treatment of Peri-Implantitis
Indication
Two or more interproximal sites with clinical ALs ≥ 4 mm (not for the same teeth) or two or more interproximal sites with PDs ≥ 5 mm , also not for the same teeth.
Non-Surgical Treatment of Peri-Implantitis
Methods
- titanium-coated or carbon-fiber curettes for calculus
- Ultrasonic instruments with non-metallic tips also for calculus removal
- rubber cups and a polishing paste for plaque removal
- topical application of antibiotics
- Lasers
- Air polishing devices
- Photodynamic therapy
Surgical Treatment of Peri-Implantitis
Indication
BoP and/or suppuration in combination with deep pockets , that remain after initial therapy
Surgical Treatment of Peri-Implantitis
Pre-requisites
appropriate standards of self- performed infection control
Surgical Treatment of Peri-Implantitis
Methods
- Mechanical debridement with carbon-fiber curettes in addition small pieces of gauze or pellets soaked in saline
- antiseptic agents and/or systemic antibiotics
- Implantoplasty
- Flap surgery, bone grafting, and guided tissue regeneration
What are the goals of surgical therapy in peri-implantitis?
- To acquire access to the implant surface
- Thorough debridement and decontamination with anti-infective measures
- Inflammatory lesion resolution and supporting bone preservation
Which is better? Surgical or Non- Surgical?
Conclusion
Peri-implant mucositis is treated by
- removing bacterial biofilm through mechanical debridement,
- improving oral hygiene, and
- prescribing antibiotics for severe cases.
Conclusion
Peri-implantitis is treated by
removing bacterial biofilm and infected tissue through mechanical debridement and surface cleaning, with surgical intervention required for advanced cases.
- Various methods and options are available for both surgical and non-surgical treatment. However, before treatment, appropriate infection control is necessary.
