Child History Need to Know
- Past History

- Growth and Developmental Milestones
RoS Head
- assess fontanel closure
- flattening due to sleeping on back
RoS Eyes
- watch that infants vision is developing
- can follow mom and dad, tracking- by 3 to 4 months, children can fixate on one visual field simultaneously
- vision testing recommended for all kids 3 – 5
- Important to catch any abnormalities early*
RoS Ears
-do they need tubes
-where the ears are located on the head- low set ears can be signs of different abnormalities
– such as renal and cognitive impairment
RoS Palate
-is it cleft?
RoS Neurologic
- Age appropriate cognition, situational awareness
* reflexes are important* some will fall by the wayside as child gets older
RoS Teeth
- first tooth comes in at around 6 mos
- first dentist visit at 6 mos
- Color and hygiene of teeth can indicate malnourishment, acid reflux, lack of fluoride in diet
- baby bottle tooth decay- caries
- < 3 y/o smear or rice sized amount of fluoridated toothpaste
- 3 to 6 y/o pea sized amount of fluoridated toothpaste
- ***formula does contain fluoride
RoS Abdomen
-look for umbilical hernias
RoS Genitalia
Tanner staging – levels according to age and where they should be puberty-wise
Questions to ask re puberty
-hair growth, onset of menstruation, breast development
RoS Nutrition
-infants need head control to start new foods
-allergies- rice cereal is least problematic
-comparison to growth chart
-Birth weight!!!
Needs change based on age
Newborns should regain their birthweight by 2 weeks old
- solids at 6 months
- cows milk at ONE YEAR
- rice cereal at about 4 months, mixed with formula
