101.3 Describe the components of the Risk Assessment matrix.
- Hazard Probability - likelihood a potential consequence may occur from a hazard
- Hazard Severity - assessment of the consequence of a potential hazard and is defined by the degree of injury, etc.
- Category:
I - May cause death
II - May cause severe injury
III - May cause minor injury
IV - Minimal threat to personnel safety
101.4 Define mishap probability and describe the four subcategories of mishap probability.
- Mishap probability - The probability that a hazard will result in a mishap or loss.
Categories: - Sub-category A - Likely to occur immediately or within a short period of time.
- Sub-category B - Probably will occur in time.
- Sub-category C - May occur in time.
- Sub-category D - Unlikely to occur.
101.5 Define RAC and list the five RAC’s.
Risk Assessment Code - Expression of risk that combines hazard severity and mishap probability.
- RAC definitions:
- 1. Critical risk
- 2. Serious risk
- 3. Moderate risk
- 4. Minor risk
- 5. Negligible risk
101.6 Discuss the timeliness and means for filing mishap investigations reports.
All Mishap investigation reports should be submitted within 30 days of mishap occurrence.
- Class A Mishap - Property damage of $2,500,000 or more
- Class B Mishap - Property damage between $600,000 - $2.5M
- Class C Mishap - Property damage between $60,000 - $600,000
- Class D Mishap - Work related injury not classified as A, B, or C mishap.
101.7 Name the four required mishap reportable items.
- Class A, B, and C government property mishaps.
- Class A, B, and C on-duty DoD mishaps military or civilian.
- Any work related injury that involves medical treatment
- Other incidents to prevent mishaps.
101.8 State the purpose of HAZREP messages.
A Hazard report to be submitted when trying to reduce community-wide mishaps.
