A

1) Fever >38.5C;<div>2) WBC > 12,000/mm3;</div><div>3) Non-weight bearing on affected side;</div><div>4) ESR >40mm/hr.</div><div><br></br></div><div>Three out of 4 criteria confers a 93% probability for septic arthritis, while 4 out of 4 criteria confers a 99% probability for septic arthritis.</div>

The image shows Gottron’s papules consistent with Dermatomuositis. Diagnosis is by muscle biopsy, and treatment is high dose steroids and rheumatology consult.
