Actinomycosis

A
  • Actinomyces israelii, G+ rods
  • characterized by abscesses and sinus tracts
  • normally reside in oropharynx, GI, vagina
  • must be inoculated to deeper tissues and have anaerobic env to cause disease
  • cervicofacial - jaw injury, dental extraction
  • thoracic - aspiration of organisms
  • abdominal - disruption of bowel, esp appendix
  • pelvic - prolonged use of IUDs
  • abscess grows slowly and becomes series of abscesses connected by sinus tracts

A
  • Nocardia asteroides
  • aerobic, G+, weakly acid fast
  • most common in pts with impaired cell mediated immunity
  • usually enters via respiratory tract
