What is the difference between Cholelithiasis and Cholecystitis?
Cholelithiasis: Stones in gallbladder
Cholecystitis: Inflammation of gallbladder
Risk factors for Gallbladder disease (SATA)
- Female
- Multiparity
- Age older than 40 yrs
- Estrogen therapy
- Sedentary lifestyle
- Genetics/ethnicity
- Obesity
5 F’s: Fat, Fertile, over Forty, Fair, Female
When does Cholelithiasis become a problem?
When the gallstones start to move causing pain (sharp)
What is Cholecystitis most commonly associated with?
Obstruction from stones or sludge
Symptoms of Cholecystitis
- Referred pain in right scapula and RUQ
- Occurs 3-6 hours after high fat meal or when a patient lies down
- Tachycardia, Diaphoresis
What does the stool look like with a total obstruction?
Clay colored stools or Steatorrhea
What does the urine look like with a total obstruction of the gallbladder?
Dark amber urine
What is the first option for treatment of Cholecystitis & Cholelithiasis?
Surgery
What is a concern with giving anticholingerics after surgery?
Dry mouth
What nutritional concern are trying to prevent after surgery?
Avoidance of rapid weight loss by giving small frequent meals with some fat, diet low in saturated fats, high in fiber and calcium
What is the number 1 complication after surgery?
bleeding
What do you do for a patient who has a total obstruction and has pruritus?
Antihistamines Non-alcoholic lotions Soft linens Short clean nails Scratch with knuckles rather than nails
Postoperative teaching
- Remove bandages after surgery and then you can shower
- Report signs of infection
- Gradually resume activités
- Return to work in 1 week
- May need low-fat diet for several weeks
- No heavy lifting for 4-6 wks
