A
  • Where the pain is and it’s distribution (shade for representation)
  • pain type anaesthesia or paraesthesis or pain)
  • quality of pain (sharpe, dull, ache, buring)
  • intensity (VAS)
  • constant vs intermittent
  • deapth
  • relationship between symptoms
  • other symptoms (joint sounds)
  • VBI signs
  • cauda equina signs
  • cord signs
