Phalen’s Test (and reverse)

A

Phalen’s Test – reverse prayer
• Flex both wrists and hold together for 60 seconds
• Flexor retinaculum increased compression of the medial nerve in the carpal
tunnel.
• + Paresthesia of median nerve
• Thumb, index finger, middle finger, and medial half of ring finger
• Indicates carpal tunnel syndrome

Reverse Phalen’s Test
• Prayer position
• Patient extends the affected wrist
• Patient grips your hand
• With opposite thumb, press on the carpal tunnel
• +Pain, Tingling may indicate compression medial nerve
• Extending the hand and pressure over the carpal tunnel further constricts the tunnel

A

de Quervain’s Tenosynovitis – stenosing tenosynovitis
• Instruct patient to make a fist while grabbing thumb
• Adduct wrist
• Stresses the abductor pollicis longus and extensor pollicis • +Pain in the distal styloid process of the radius indicates
• + Pain LU7 LU9 LI5 abductor pollicis longus, extensor policis brevis • Needle LU7→LI5
