Furosemide (Lasix)

Onset: 5-10 minutes (vasodilation), 5-30 minutes (diuresis)
Peak effects: 30 minutes (vasodilation), 20-60 minutes (diuresis)
Duration: 2 hours (vasodilation, 6 hours (diuresis)
Half-life: 30 minutes

MOA: A loop diuretic that inhibits the reabsorption of both sodium and chloride in the kidneys. It is used in CHF and pulmonary edema. Has two effects: (1) causes venous dilation, thereby causing a reduction in preload (decreasing cardiac workload). (2) Diuresis.

Class: diuretic

Description: a potent diuretic that inhibits sodium and chloride reabsorption in the kidneys and causes venous dilation.

Indications: CHF, pulmonary edema

Contraindications: pregnancy, dehydration, known allergy to sulfa medication class

Precautions: protect from light

Side effects: headache, dizziness, hypotension, volume depletion, potassium depletion, arrhythmias, diarrhea, N/V

Interactions: should not be used in same line as inamrinone (Inocor), due to crystalization. Do not use with other diuretics.

Route: IV

Adult dosage: 40-80 mg (if they are already taking chronic furosemide therapy, double their home dose)

Peds dosage: 0.5-2 mg/kg/dose

Bumetanide (Bumex)

Onset: 1-2 minutes
Peak effects: 15-30 minutes
Duration: 3.5-4 hours
Half-life: 60-90 minutes

MOA: A loop diuretic that inhibits the reabsorption of sodium chloride in the kidneys; 1 mg of bumetanide has the diuretic potency of 40 mg of furosemide.

Description: a potent diuretic with a rapid rate of onset and a short duration of action

Indications: CHF, pulmonary edema

Contraindications: pregnancy, dehydration

Precautions: can cause dehydration and electrolyte depletion; can be used in pts who have a furosemide allergy

Side effects: muscle cramps, dizziness, hypotension, headache, N/V

Interactions: can potentiate the effects of antihypertensive agents… use caution

Route: IV, IM

Adult dosage: 0.5-1.0 mg

Peds dosage (less than 6 months): 0.015-0.1 mg/kg/dose
