Causes of Spontaneous Abortion?

A

Maternal - Infections, Trauma, Endocrine, maternal malnutrition
Fetal - chromosomal abnormalities (70%), faulty implantation of the embryo
Placental - Previa, Abruption

A

<12 weeks - Early Abortion - Elective and therapeutic
>12 weeks - late Abortion - only Therapeutic
>20 weeks - Stillbirth

  1. Elective - <12 weeks, at request of patient. NOT due to maternal risk. BEFORE Fetal viability
  2. Therapeutic - if mothers life is at risk. Rape/incest. If risk of child with severe physical deformities

Causes of Septic Abortion. CP. Treatment

A
Pathogens from vaginal + bowel flora 
Usually due to criminal Abortion 
CP
Pyrexia, malaise, sweating. 
POC may be retained - oxytocin given - > expulsion of remaining contents. 
Treatment - Abx, fluids, bed rest

What are the complications of Abortion?

A

Immediate (<3 hours) - bleeding, cervical injury, uterine perforations
Delayed (>3 hours) - Infection, Ashermans syndrome (scarring of uterine cavity - leads to menstrual abnormalities and infertilisation), Rh sensitization
