Acute- chest syndrome, anemia, cholecystitis, infection, spleen sequestration, multiorgan failure
chronic- leg ulcer, bone death, gall stones, pain, retinopathy

blood transfusion with goal >10, monitor iron overload
- chelation therapy to reduce iron overload
