(0.4-1.0 mmol/L) - taken 12 post-dose, weekly after dose change until stable, then every 3m for a year, then 6m

S/E
- n/v/d
- fine tremor
- polyuria
- weight gain

serious
- hypothyroidism (low mood, reduced appetite …)
- intracranial HTN
- nephrotoxicity - neph diabetes insipidus (polyuria)
- leucocytosis (fine if asympt)
- hyperPTH - hyperCa
- ECG: T wave flattening/inversion

  • so check TFT, ECG, U/E, FBC,
thyroid and renal every 6m longterm

Q

first line SSRi
- in adults
- breast feeding
- in paeds

Q

serotonin syndrome

  • causes
  • symtpoms
  • treatment
A
  • monoamine oxidase inhibitors
  • SSRIs
    — St John’s Wort, often taken over the counter for depression, can interact with SSRIs to cause serotonin syndrome
  • ecstasy
  • amphetamines

(1) neuromuscular excitation (myoclonus and hyperreflexia)
(2) autonomic nervous system excitation (fever and sweating)
(3) altered mental state (confusion and agitation)

IV fluids
benzos
cyproheptadine and chlorpromazine (Serotonin antagonists if severe)

Q

neuroleptic malignant syndrome

  • causes
  • symptoms
  • mx
A

antipsychotics

young patients
tachycardia and tachypnoea, hypertension, fever, sweating.
muscle rigidity rather than myoclonus, hyporeflexia

IV fluids
Benzos
dantrolene (severe)

17
Q

differences between neuroleptic malignant syndrome and serotonin syndrome

  • causes
  • symptoms
  • mx

[recap* - more cards explaining if this isnt enough]

A

antipsychotics vs SSRI/MAOi/ ectasy / amphetamines

hyporeflexia vs hyperreflexia with serotonin syndrome
more rigidity than myoclonus in neuroleptic

severe treatment varies - dantrolene vs cyproheptadine and chlorpromazine

18
Q

Gamma glutamyl transferase (γGT) indicates what

A

raised = suspect alcoholic

19
Q

alcoholic markers in blood

A

Gamma glutamyl transferase (γGT)

dehydration - raised urea and creatinine

nutrition - low albumin

LFTs :/

20
Q

ekbom syndrome

A

Formication - bugs under skin

21
Q

De Clerambault syndrom

A

erotomania - often believes famous person in love with them
(link to paranoia?)

22
Q

De Frégoli syndrome

A

a stranger is a familar person (in disguise)

23
Q

how long on antipsychotics for
- neuropleptic malignant syndrome
- acute dysstonia
- tardive dyskinesia

A

short
short
long

24
Q

akathisia treated by what

A

propanolol

25
Q

what should be monitored starting
- SNRI
- SSRI
-citalopram

A

SNRI - BP (HTN)
SSRI- U/E (hypoNa)
citalopram - ECG
