16
Q

Describe Rest position (a basic mandibular position)

A

The vertical and horizontal position , when the mandibular musculature is relaxed and the patient is upright

There is a space between the occlusal surfaces of the teeth= Freeway space=Interocclusal rest place (2-4 mm)

17
Q

Describe CR - Centric relation (a basic mandibular position)

A

The mandibular jaw position in which the head of the condyle is situated as far anteiorly and superiorly as it possible can within the mandibular fossa

18
Q

When should we use CR - Centric relation?

A

when restoring edentulous patients with removable, implant-fixed or fixed prostheses

19
Q

Why is CR - Centric relation important?

A

Important the reproducibly relate the patient’s maxilla and mandibl

20
Q

What is Intercuspal position?

A

( CO-Centric occlusion)
It is the vertical and horizontal position of the mandible in which maximum occlusal contact occurs

21
Q

Describe Rest vertical dimension (RVD) (a basic mandibular position)

A

When mandible is in its resting position, this distance is RVD.

The distance between two selected points:
1. Related to the maxilla (Nasion)
2. Related to the mandible (Gnathion)

22
Q

Describe Occlusal vertical dimension (OVD) (a basic mandibular position)

A

When the upper and lower teeth are in contact is known as the OVD.

The diffference between RVD-OVD=Freeway space

23
Q

What is Freeway space?

A

The diffference between RVD-OVD

24
Q

Characteristics of Freeway space

A

1/ In rest position of the mandible
2/ An excessive freeway space is provided, the load of the tissues is of course reduced
3/ 2- 4 mm

25
Q

How to do CO (Centric occlusion) registration?

A
  1. Measuring of vertical dimension
  2. Measuring of horizontal dimension
(With the help of occlusal rims)
26
Q

How to measure the rest RVD ( Rest vertical dimension)

A
  1. Patient need to be calm
  2. Without external stimuli, no disturbing noises or bright light shining in the patient’s eyes
  3. Swallowing, make sounding M before trying to relax the mandible
27
Q

Measurement of the vertical
dimension

A
  1. Try in of the occlusal rims and plates
    - RVD is measured as the distance between 2 selected points (Maxilla, mandible)
    - The two-dot technique Willis gauge
  2. Measurement of OVD
28
Q

What type of cast is used for measurement of the vertical dimension?

A

Funtcional cast

29
Q

Name of these 2 points when doing Measurement of the vertical dimension

A

Signing of Gnathion-Nasion points

30
Q

Adjusting of upper occlusal rim
-> What should we do before adjusting?

A

any adjustement upper and lower record blocks should be placed in the mouth in turn and checked for

31
Q

What are the 2 basic objectives of adjustment of upper occlusal rim?

A
  1. Two basic objectives
To establish the correct orientation and level of the occlusal plane
  2. To produce the correct shape for the labial, buccal and palatal surfaces
32
Q

How is the rim adjusted during adjustment in the coronal plane?

A

The rim is adjusted so that the occlusal plane is parallel to the interpupillary line

33
Q

How is the rim adjusted during adjustment in the sagittal plane?

A

A useful guideline for saggital orientation of the occlusal plane – is the ala-tragal line

34
Q

How do we establish the level of the occlusal plane?

A
  1. Establishing the level of the occlusal plane
  2. The incisal level of the upper rim is related to the upper lip
  3. Short lip- long upper lip
  4. Modifying the incisal edge of wax rim
  5. Controlling! During function by smiling, saying few words while wearing the record block
35
Q

How do we shape the labial and buccal surface?

A
  1. Lip supports depends upon the position and inclination of the labial face of the wax rim
  2. Palatal surface: Essential to create adequate space for the tongue
36
Q

How do we adjust the lower record rim?

A
  1. Adjusting the height of the rim to the required occlusal vertical dimension
  2. After the establishing oft the upper occlusal rim , the lower rim will be governed by it
  3. Fitting them into the required occlusal vertical dimension OVD
37
Q

Disadvantages of non fitting lower occlusal rim

A
  1. Unstable lower rim
  2. Displacing of the cheek and lip mucosa
  3. Not enough place for tongue
38
Q

Why is Horizontal dimension important?

A

Recording the retruded jaw relationship

Active, Myodynamic, Passive

39
Q

Why is Definitive recording important?

A
  1. Deciding whether the facial proportioners look correct, if the lower third of the face appears too long or too short
  2. Assessing speech with the blocks
  3. Reference to previous dentures
40
Q

Definitive recording
-> Before finally fixation of occlusal rims, what should we do?

A
  1. Are the rims stable?
  2. Is there adequate freeway space?
  3. Occlusal contact?
  4. Is there a pleasuring appearance?
  5. Has a consistent retruded jaw relationship
    been established?
  6. Spatula try-in
