Immunization Records

A

~ the child care program must obtain a current, complete, and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680 for each child in care, except for school-age children
~ a child may be exempt from providing immunization records to the child care program if they provide the proper documentation to the program
~ if the immunization is from out of state, it must be transferred to the Florida Certificate of Immunization form and signed by a Florida physician
~ if families need assistance with these requirements, the program should refer them to the Department of Health or their physician
~ medical records are the property of the families and must be returned if the child withdraws so they can provide them to another program
~ if the parent or guardian fails to provide the required documentation within 30 days of enrollment, the child should be excluded from care
~ if the child has not received the age-appropriate immunizations prior to enrollment, or has an exemption, the family must provide documentation of a scheduled appointment or arrangement to receive immunizations
~ providers must notify the families in writing, at the time of enrollment, that some children in care may not have current immunizations
