What is the pathophysiology/potential causes of BMS?

A

Salivary Gland Dysfunction - subjective xerostomia and relief in drinking water but no evidence of saliva composition change

Peripheral and CNS abnormalities - dysgeusia, elevated taste thresholds (chorda tympani dysfunction), lower density epithelial nerve fibres ant 2/3 tongue, reduced dopamine levels

Psychosocial issue - anxiety, depression and stress

What are the clinical characteristics of BMS?

A
  • Post menopausal women
  • Tongue most common site
  • Burning that appears suddenly with no known precipitating incident and is constant
  • Emotional stress can worsen burning
  • Altered taste sensation (metallic or bitter)
  • Preceding diagnosis of anxiety or depression
