What do you check during a complete denture try in appointment before the patient arrives?
- do I have the lab work
- is it for the correct patient
- any damage to lab work
- does it match prescription
- mould & shade okay
Before you insert the try-in of a complete denture, what checks should you do?
- are the try in bases stable on the cast
- are the borders smooth, rounded and with no sharp edges
- is the border extension shaped to the depth and width of the functional sulcus
If at the try-in stage of a complete upper denture a post dam has not been cut, what should you don?
- draw on cast with pencil
- prescribe for the technician to cut post dam
When you put the try-in complete dentures in the mouth what is to be expected with reference to retention & stability?
NOT AS GOOD at try-in as with finished denture
- make sure to explain this to the patient
If you put in a try-in upper complete denture and hold the tissues away from the denture & it drops, what may this suggest?
Possible under extension into sulcus/undercuts
If the try-in complete denture is overextended in the peripheries how will you know?
When you manipulate tissues it will drop/displace
At a complete denture try-in stage, what must you check?
- retention & stability
- base extensions
- LIMBO
- neutral zone (position of teeth compared with soft tissues)
- free way space
- ensure even contact in occlusion
- speech & aesthetics
If the teeth make contact during speech and cause speech problems in a complete denture try-in, what should be done?
Insufficient FWS so the OVD needs to be reduced
A patient is at the try-in stage of complete dentures, when they speak with the dentures in there is a significant whistling sound, what is wrong and how can this be fixed?
Too much air escaping
- OVD may need to be increased
- or anterior tooth position changed
If there is a minor overextension at the complete denture try-in stage, what should you do?
- modify wax
- mark on master cast maximum extent of flange
- give specific lab instructions
If there is a major overextension at the complete denture try-in stage, what should you do?
go back to master impression stage
If there is a minor underextension at the complete denture try-in stage, what should you do?
Two options
1. wash impression in try in —> lab for casting/mounting —> re try-in
2. proceed to insertion stage but do not insert at at that stage instead reline denture
If there is a major underextension at the complete denture try-in stage, what should you do?
go back to master impression stage
Where do patients commonly find discomfort with complete dentures?
- undercut areas
- areas of poor support
What wear instructions should be given to the patient when you give them their complete dentures?
- some discomfort is normal
- speech may lisp for a short while
- eating may be difficult
- aesthetics change
- wear as much as possible!
