Explain the considerations for lung transplantation with history of malignancy.

A

Lung transplantation should not be offered to adults with a recent history of malignancy. A 2-year disease-free interval combined with a low predicted risk of cancer recurrence after lung transplantation may be reasonable, for instance, in non - melanoma localized skin cancer that has been treated appropriately. However, a 5-year disease-free interval is prudent in most cases, particularly for patients with a history of hematologic malignancy, sarcoma, melanoma, or cancers of the lung, breast, bladder, or kidney.

What are the BMI cutoffs for lung txp?

A

BMI >35 is an absolute contraindication.
BMI 30-35 is a relative contraindication.
BMI 25-30 is still assd w/ a 2x risk of severe PGD.

Weight loss shows improved outcomes.
