A
  • Identify the person who died
  • Date and place of death
  • Manner and cause of death

When should I report to the coroner?

A

Cause of death - Unknown, unexpected, unnatural or violent, suspicious or unusual
Nature of death
- Resulted directly or indirectly from an accident or injury
- During or as a result of anaesthetic, surgical procedure, invasive medical or diagnostic procedure
- Not a reasonably expected outcome of a healthcare procedure.
- Within 24 hours of discharge from a hospital as an inpatient or having sought emergency treatment at hospital.
In police or other lawful custody.
Held in care - Mental health facility or residential service.
