what are the maternal assessments and observations that you need to undertake in the immediate post partum period? include rationale

A

Assess perineum and repair if required - as soon as practicably possible to minimise infection and blood loss

Assess blood loss and ongoing bleeding - treat for PPH if required

Assess uterine tone and fundal height (at umbilicus) - to ensure adequate involution of uterus, distended uterus could indicate a full bladder, retained tissue or inadequate uterine tone.

Maternal observations (BP, pulse, temp, resps, o2) - check for signs of infection or BP may be low if haemorrhaged.

Pain relief as indicated - to help control pain (perineal and pelvic floor, after pains)

Bladder care - IDC if required for epidural or genital tract trauma, to ensure urinary retention does not affect uterine involution.

continue to support and facilitate skin to skin for mum and baby

give perineal care advice
