Answer 1

obs 1 additional grade should be added for serious systemic disease - HTN, DM, severe atherosclerosis, COPD or severe vasospasm on angio
1. asymtomatic/mild H/A/ slight nuchal rigidity
2. CrN palsy(III or VI), moderate/severeH/A, nuchal rigidity
3. mild focal deficit, lethargy, or confusion
4. stupor, moderate to severe hemiparesis, early decerebrate rigidity
5. Deep coma, decerebrate rigidity, moribund appearance

0= INTACT ANEURYSM
1- GCS 15
2- GCS 13-14
3- GCS 13-14 + either aphasia, hemiparesis or hemiplegia.
4- GCS 7-12 w/W/O major focal deficits
5- GCS 3-6 w/w/o major focal deficits.
