1
Q

what is malignant hyperthermia?

A

a life threatening elevation in body temp after inhalation of general anesthetics (halothane) or muscle relaxants (succinylcholine)
*the patient does not resequester Ca2+ in malignant hyperthermia

2
Q

s/s of malignant hyperthermia

A
  • fever
  • increased boy temp
  • hyperhydrosis
  • hyperkalemia
  • metabolic/resp acidosis
  • tachypnea
  • tachycardia
  • muscle spasms
  • arrythmias
3
Q

how do you treat malignant hyperthermia?

A
  • stop administration of anesthesia
  • hyperventilate with 100% O2
  • correct acidosis
  • cool patient (ice bath)
4
Q

what medication can be administered to reverse effects of malignant hyperthermia?

A

dantrolene

-1mg/kg rapid IV push until symptoms subside

5
Q

What is normal body temp?

A

97.7-99.5F

or 36.5-37.4C

6
Q

what is febrile temp?

A

> 100.4 F (or 38C)

7
Q

what is heat pyrexia and when does it occur?

A

occurs during surgery and up to 1 hour post-op

8
Q

what is the cause of heat pyrexia?

A

trauma of procedure resulting in inflammation (cytokines released) ; increased permeability and prostaglandins

9
Q

what are some causes of post-op fever?

A
(rmr mnemonic- wind, water, wound, walking, wonder drugs)
wind: atelectasis
water: UTI
wound: wound infection
walking: DVT/ thrombophlebitis
wonder drugs: drug fever
10
Q

when does post-op fever due to atelectasis occur?

A

POD 1-2

11
Q

when does post-op fever due to UTI occur?

A

POD 2-3

12
Q

when does post-op fever due to wound infection ocur?

A

POD 3-7

13
Q

when does post-op fever due to DVT/thrombophlebitis occur?

A

POD 5-7

14
Q

when does post-op fever due to drugs occur?

A

POD > 7

15
Q

causes of anytime fever

A

drug fever
catheter fever
reaction to blood transfusion

16
Q

there are two types of post-op edema. what is the 1st type?

A

short duration (15-30 min) caused by factors that increase permeability

17
Q

there are two types of post-op edema. what is the 2nd type?

A

long duration (1-2 days) from leakage caused by endothelial injury
