what is an adverse event?

A

results in unintentional harm
- errors of commision:
- errors of omission?
- failure of doing something that should have been done.
- medication that was supposed to be given but was not.

A

action that was not completed, as intended
- patient had an allergy to PCN and was given it and had a reaction
- also called never event (event that should have never happened)

- medication that was ordered supposed to be given but was not.
