A

Comprehensive nutritional assessment-dietary history and clinical information, physical exam for clinical signs, anthropometric measures, and lab tests
24-hour recall-asked to recall everything they ate in the last 24hours
Food frequency questionnaire- collect how many times per day, week, or month the individual eats particular foods
Food diaries- have pt right everything they eat
Direct observation- making observations while assessing

Signs of malnutrition

A

Skin- dry, flaking, scaly, dry, eczema, xanthomas
bumpy, cracks on hands legs face, or neck
Hair-dull,dry, color changes, corkscrew hair
Eyes-foamy plaques, dryness, softening, pale/red conjunctivae, blepharitis
Lips-vertical cracks, red cracks at side of mouth
Tongue-beefy red, pale, papillary hypertrophy/atrophy, megenta/purplish colored
Gums-bleeding
Musculoskeletal-pain in calves, thighs, osteomalacia, rickets, joint pain, muscle wasting
Neurological-peripheral neuropathy, hyporeflexia, disorientation/irritability
