Psychopharmacology
Degree of impairment compared to maximum of 0.04% BAC is considered safe
1st generation antidepressants
Absolute DQ (high s/e profile)
amitryptyline (elavil)
imipramine (tofranil)
2nd generation antidepressants
limited s/e
wellbutrin
prozac (fluoxetine)
zoloft (sertraline)
effexor (venlafaxine)
max cert = 1 year
antipsychotics
usually tx for schizophrenia, mood disorders, personality disorders
neuroleptics can cause variety of AE
max cert = 1 year –> depends on ME and whether sx interfere w/ driving
anxiolytics and sedative hypnotics
max cert 2 years if
–benzo/hypnotic is short acting = 1/2 < 5 hours; lowest effective dose for < 2 weeks
–any non-sedating anxiolytic (buspirone, hydroxyzine)
Lithium
usually used for BPD and depression
little evidence it effects ability to drive
max cert 2 years
ECT
WP = 6 months s/p last ECT course
cannot be undergoing maintenance therapy
cyclothymia
mild form of BPD –> brief episodes w/out marked impairment
BPD + depression
certification not based on dx alone
ability to drive safely and effectively is assessed w/ focused eval of fx and relevant hx
WP
—6 months for non-psychotic major depressive episode w/o suicidal behavior
— 1 year for severe episode, suicide attempt, or manic episode (must report w/in 30 days of onset)
personality disorders
max cert = 1 year
Alcoholism
ME not responsible for drug & alcohol testing, but can assess for s/s alcoholism –> can request testing based on reasonable suspicion (behavior, speech, body odor)
assess on PE –> enlarged liver, tremor
