What is circadian rhythm?
physical, mental and behavioural changes that follow a daily cycle
How is cortisol produced?
Hypothalamus produces CRH -> pituitary releases ACTH -> adrenal gland produces cortisol
Define adrenal insufficiency (AI)
A condition in which the adrenal glands do not produce adequate amounts of steroid hormone (mainly cortisol)
What is primary AI?
Addison’s disease
What is secondary AI?
Hypopituitarism
What is tertiary AI?
Suppression of HPA
What are symptoms of AI?
fatigue, weight loss, poor recovery from illness, adrenal crisis, headache
What past history may show AI?
TB, post partum bleed, cancer
What family history might show AI?
Autoimmunity, congenital disease
What would you treat AI with?
Any steroid
What are signs of AI?
- Pigmentation and pallor
- Hypotension
What biochemistry shows AI?
- Low Na, high K (SIADH like picture)
- Eosinophilia
- Borderline elevated TSH
What investigations would you do to investigate AI?
- 0900 cortisol and ACTH
- Renin/ aldo
- Synacthen test
What cortisol result signifies AI?
Cortisol < 100 nmol/l
What ACTH and cortisol results signify primary AI?
- ACTH > 22 pmol/l
- Elevated renin
