Monitor Officer
A designated officer who is NOT involved in the use of deadly force and whose responsibilities are to PREVENT DISCUSSIONS REGARDING THE INCICENT AMONT SUBJECT OFFICERS, WITNESS OFFICERS, WITNESSES, AND OTHER INDIVIDUALS WHO ARRIVE AT THE SCENE PRIOR to Force Investigation Team (FIT).
Officer Involved Shooting
An officer’s discharge of a firearm at a person, with or without physical injury.
Reportable Force
Any UoF which results in INJURY or COMPLAINT OF CONTINUING PAIN, or ANY USE OF FORCE GREATER THAN LOW-LEVEL FORCE.
UoF reports WILL be completed by ___. Any exceptions MUST be approved by a supervisor and reviewed through the chain of command within ___ days.

Levels of control that require the completion of a UoF report (3)
- Low-level force that results in INJURY, COMPLAINT OF INJURY, or COMPLIANT OF CONTINUING PAIN.
- All intermediate force.
- All deadly force

Non-deadly force requiring a UoF reports WILL be investigated by the OFFICER’S CHAIN OF COMMAND.

Acting supervisors are NOT authorized to complete this investigation.

Supervisors involved as subject officer in a reportable UoF WILL NOT investigate themselves or other subject officers. Investigative duties will be relinquished to the next lever of supervision in the officer’s chain of command.

If the next level of supervision in the officer’s chain of command is unavailable, the supervisor involved in the UoF incident WILL NOTIFY THE ON DUTY WATCH COMMANDER. The watch commander WILL DELEGATE the next level of supervision responsible for the UoF investigative duites.

ALL reportable UoF on a subject, REGARDLESS of visible injury, WILL BE PHOTOGRAPHED and DOWNLOADED into blue team.
Non-deadly UoF Officer WILL: (5)
- IMMEDIATLEY request dispatch to notify a supervisor.
- Complete the UoF report BEFORE THE END OF THEIR SHIFT. Any exceptions MUST be approved by a supervisor.
- The report WILL include a detailed description of the level of force, tool, or tactic used, to INCLUDE it’s effectiveness, AND the eight “objective reasonable” force factors.
- Each application of force must be separately justified in the documentation.
- Forward reports to investigating supervisor, sending a copy to their immediate supervisor.
Non-deadly UoF When ECD is used, Officer WILL: (2)
- Handle the probes in the same manner as CONTAMINATED NEEDLES in accordance with biohazard disposal procedures & impound ALL PROBES, WIRES, AND CARTRIDGES AS EVIDENCE.
- Provide ECD to a supervisor for DATA DOWNLOAD.
Non-deadly UoF when using vehicle Officer WILL: (3)
- Blocking: ANY CONTACT with damage WILL be reported as a TRAFFIC INCIDENT on a vehicle incident report LVMPD 42. A UoF report is NOT required.
- VCT requires UoF report.. ANY CONTACT or COLLISION WILL be documented by the Traffic Bureau on a vehicle incident report. (only SWAT & MV Units).
- PIT: Traffic officers WILL respond to assist with the traffic investigation.
Supervisor UoF Investigative Responsibilities: (9)
- Respond, WITHOUT DELAY, to an incident in which reportable force is used.
- Determine the level of force: IF deadly force was used OR if there is substantial bodily injury (SBI), see section 5. SBI Example: skeletal fractures, brain bleeds, head/chest injuries which create a risk of death or paralysis. Wound NOT amounting to SBI may include: broken wrist, cracked tooth, abrasions or sprains.
- Conduct an investigation by interviewing the subject, officers, medical personnel, and witnesses.
- Ensure photographs are taken and placed into blue team, to include the SUBJECTS, SCENE, OFFICERS INVOLVED, and COLLECT ANY EVIDENCE.
- Review BWC. See LVMPD 7.136 BWC.
- Check the surrounding area for any video surveillance, obtain a copy, and ENSURE the third-party video is IMPOUNDED as evidence and noted in blue team.
- Review all applicable reports to ensure ACCURACY and COMPLETENESS.
- Document how a UoF has been determined to be appropriate and in adherence to department policy. If the UoF is determined to NOT be appropriate, IT WILL be sent through the CHAIN OF COMMAND to decide if a SOC is necessary. IF other misconduct is identified, THE SUPERVISOR WILL DOCUMENT HOT IT WAS ADDRESSED.
- Document training deficiencies and HOW they were corrected.
Supervisor UoF Investigative Responsibilities: ECD is used (3)
- ENSURE the data record has been uploaded to blue team.
- Verify the PROBES, WIRES, and CARTRIDGES are properly impounded, and arrange for REPLACEMENT CARTRIDGES. Accidental discharges WILL NOT REQUIRE IMPOUNDING unless there has been an INJURY.
- Document the investigation in a Citizen Contact (CC) in blue team when ALLEGED UoF is DETERMINED TO BE UNFOUNDED.
