Albumin
Dose:
Adult:
5% albumin: 500 to 1000 mL IV, (IO).
25% albumin: 50 to 200 mL IV, IO
Pediatric:
5% albumin: 0.5 to 1g/kg IV; the initial dose may be repeated in 15 to 30 minutes if the clinical response is inadequate. 25% Albumin: 1G/Kg IV. IO
Indications:
Severe anemia or cardiac failure in the presence of normal or increased intravascular volume
Hypertonic Solution
Dose:
Adult: 100 to 200mL/hr.
Ped: 0.1 to 1mL/kg per hour.
Indications:
Reduction if increased intracranial pressure resulting from TBI, hypovolemic shock.
Sod1uM Bicarbonate (NaHCO3)
Dose:
metabolic acidosis during cardiac arrest: Adult = 1mEq/kg slow IV, IO; may repeat at 0.5 mEq/kg in 10 minutes.
Pediatric = Same as adult.
Metabolic acidosis not associated with cardiac arrest: Adult and Pediatric = Dosage should be individualized.
Indications: Acidosis, drug intoxications (barbiturates, salicylates, methyl alcohol)
Insu.1in
Diabetic Ketoacidosis:
Adult – 0.1 U/kg IV. IO, or subcutaneous.
IV, IO insulin has a very short half-life; therefore IV, IO insulin with an infusion is not that effective. The rate for insulin fusion is 0.05 to 0.1 U/kg per hour IV, IO, when dosing insulin, use a U-100 insulin syringe to measure and deliver the insulin.
Hyperkalemia:
Adult: 10 U IV, IO of regular insulin (insulin R), coadmin with 50 mL of D50W over 5:00. Peds: 0.1 U/kg insulin R IV, IO
Indications: Hyperglycemia, insulin-dependent diabetes mellitus, hyperkalemia.
Calcium Gluconate/Chloride
Supplied as 10% solution; therefore, each milliliter contains 100mg of calcium gluconate.
Adult: 500 to 1000 mg IV, IO admin slowly at a rate of 1 to 1.5 mL/min; max dose 3 g IV, IO. It may also be ordered as 60 mg/kg per dose.
Peds: 60 to 100 mg/kg/ IV, IO slowly over a 5 to 10 minute period; maximum dose 3 g IV, IO
Indications: Hyperkalemia, hypocalcemia, hypermagnesemia
KCl (potassium chloride)
Adult = must be individualized according to patient serum potassium concentration.
Peds = Same.
Indications: Hypokalemia
