What two things must you ALWAYS WEAR ON ROTATIONS?
- Name badge (ALWAYS, even if you get a site badge)
- Short white coat with UCPAP logo (unless you’re like in the OR or your preceptor says no)
How many piercings are allowed to be visible?
1 single ear piercing

If you are going to be absent for a day on rotation, how soon should you document it?
Within 24 hrs
Notify site/preceptor PRIOR to absence
When do you contact your rotation site for scheduling and stuff?
2 weeks prior to a rotation!
When do you need to finish any orientation for your rotation site by?
Prior to your first day at that site
when is the mid rotation evaluation completed
by the end of the second week of each rotation
How is the end of rotation evaluation submitted
one of two ways:
1. directly to the DCE by the preceptor
2. by the student in a sealed envelope with the precptors signature across the seal
Which patient encounters do you need to log into Exxat?
ALL OF THEM
when are patient encounter logs due to be logged into Exxat/Prism
asap but no later than Sunday at midnight at the end of that weeks rotation.
What information is included for a logged patient encounter?
- Rotation & preceptor
- Type of encounter
- Setting
- Patient demographics
- Dx code
- Level of assessment/decision making
- Procedures done
Who signs off on your procedure log?
required diagnostic and technical skills

What 3 things must be done in order to finish a rotation and pass it?
- Finish EOR Site Eval
- Finish EOR Preceptor Eval
- Pass EOR with a 75%
evals completed electronically on prism/Exxat
You are injured during a rotation at the clinical site. Who do you contact and how soon?
- Notify your preceptor immediately
- Notify Davis within 48h
