Indications for MAOIs?

A

Rarely used; only in its unresponsive to other drugs/ECT

MAOI MOA?

A

Irreversibly inhibits MAO-A and MAO-B in nerve terminals.

  • MAO-A: metabolizes NE, 5HT, tyramine
  • MAO-B: metabolizes DA

A
  • Acetylators (susceptible to pharmacogenomics)

- Long duration of drug effects

Clinical effects of MAOIs?

A

Acute: CNS stimulation & euphoria
Chronic: clinical symptoms improve but CNS stimulation remains

Drug drug interactions?

A
  • Tyramine (Cheese).. Watch out for depressed person at a wine & cheese party –> sudden malignant HTN
  • Meperidine/Dextromehorphan: high temp, delirium, death
  • SSRI, Cocaine
