1st Objective - DSM Flashcards
(24 cards)
DSM - 5
the official psychiatric coding system used in US.
o Diagnostic and statistical manual of mental disorders
o Developed by the American psych assoc.
o NO multiaxial systems
o NO GAF (global assessment function)
ICD-10
Official medical classification used in US and other parts of the world
o The international classification of disease, 10th edition
o Developed by the world health organization
o Used to track mortality (death) and morbidity (sickness
Relationship between DSM-5 and ICD-10
- DSM-5 was designed to correspond with ICD10, the purpose was to ensure uniform reporting of national and international health stats
- ALL CATEGORIES IN DSM-5 are found in ICD-10 BUT NOT all ICD-10 categories are in the DSM-5.
Organization of disorders in DSM-5
Early lifespan → adolescent and young adult → adult and later life
Early lifespan (disorder and group)
o Neurodevelopment and schizophrenia spectrum
o Internalizing disorders (emotional & systemic)
(NDSI)
Adolescent and young adult (disorder and group)
o Anxiety, Bipolar, Depression Disorder.
o Externalizing disorder
(ABDE)
Adulthood and later life (disorder and group)
o Neurocognitive disorder
same for the group
Organization of group in DSM-5 helps with what?
o The hope is that this organization will encourage non-mental health providers to identify potential diagnoses earlier
o This encourages further study of the underlying pathophysiological process that causes the diagnostic comorbidity & symptom heterogeneity
The appropriate action of the DSM-5 in clinical practice
o Careful clinical history (OPQRST)
o a concise summary of social, psychological and biological factors.
Elements requried to meet a metal disorder
o THINK: the amygdala and prefrontal cortex. o Cognition o Emotional regulation o Behavior (BEC)
Mental disorders are usually associated with significant distress or disability in
o Social activities
o Occupational activities
o Other important activities
Evaluate pts situation be sure to be able to distinguish between…
o The response to the loss of a loved one, given the expectable and cultural environment, is NOT a mental illness
o A medication induced psychosis is NOT a mental illness
Diagnostic Criteria
offers guidelines for making decisions, their use should be in connection to clinical judgment
Text descriptors help support…
the diagnosis and provide a differential diagnosis.
When should you consider the application of disorder; subtyped and specifies.
ONLY when the full criteria are met
When criteria are not met you…
determine if the symptoms present meet criteria for another specified or unspecified diagnosis
Four final diagnosis components
- Clinical interview
- Text descriptions
- Criteria
- Clinical judgment
Subtypes are…
mutually exclusive and jointly exhausting phenomenological subgroupings with a diagnosis
Specifiers are…
not meant to be exclusively or jointly exhaustive, one or more specified may be given.
Specifiers can indicate
- Can indicate course as in partial remission of full remission.
- The severity of the pt. at the time of diagnosis which means rating the intensity, frequency, duration, and symptoms count
Principal Diagnosis: Inpatient
the condition established after study to be chiefly responsible for the admit
Principal Diagnosis: Outpatient
the reason for the visit (the cc) is the principle diagnosis. This diagnosis is attached to any service given to the pt during that visit.
If the pt has more than 1 diagnosis than….
the principle diagnosis is listed first and the remaining disorders are listed in the order of need focus of attention and treatment.
Provisional Diagnosis
o Used then there is a strong presumption that the full criteria will ultimately be met for a disorder but not enough information is available to make a firm diagnosis.
o Can be used when the full spectrum of depressive disorder is not met due to the time frame and depressive disorder depends exclusively on the duration of the illness.
