Check, explain, wash steps:
identify patient by checking arm band or area-specific procedure.
Explain the procedure to the patient
Wash hands for 15 seconds, or use alcohol based hand rub
After washing your hands what must yo do to the blood culture bottles?
Prep the rubber cap with an alcohol pad in a circular motion, allow to dry.
How do you prep the puncture site (when doing blood cultures)?
Prep using CHLORHEXIDINE
Using aseptic technique, remove the applicator from its package.
Hold downward, gently squeeze the wings to release the solution.
Scrub with a back & forth motion 30 seconds on dry skin or 2 minutes on wet skin.
Do not wipe that site after cleansing.
After you’ve washed your hands & prepped the puncture site with chlorhexidine, whaddya do next?
Apply gloves.
Sterile gloves if you want to palpate site your puncturing, nonsterile if you will not palpate that site
What size syringe (ml) is needed when drawing blood on:
Adults, Pediatric, Infants?
Adult: 20 ml
Peds: 20 ml
Infants: 3 ml
How much blood is required for blood cultures (same for anaerobic/aerobic) in:
Adults, Pediatric, Infants?
Adults: 10 ml
Peds: 2.5-10 ml
Infants. 0.5-1 ml
Do not add more than __ mL of blood to each bottle
no more than 10 ml. Do not overfill bottles.
What do you do if you cannot obtain 20 ml of blood (one for each culture bottle), and only obtain <10ml?
Put it in the aerobic bottle. That one is examined first, then the blood is used in the anaerobic bottle.
After blood is in the culture, whaddya do? (3)
Mix, gently
Then label with site & specimen collection (not covering bar codes, don’t be stupid and hasty)
Then it’s sent to the lab
After the blood’s sent to the lab, whaddya do?
Document: date & time, site of specimen
If 2 sets of blood cultures have been ordered, whaddya do?
Same manner as first, making a new venipuncture at a different site.
