16
Q

What joint is most commonly affected by gout?

A

Metatarsal-phalangeal joint at the base of the big toe.

17
Q

What are the causes of hyperuricaemia?

A

Purine rich diet (i.e. beer).
Increased purine synthesis (i.e. psoriasis).
Decreased renal excretion.

18
Q

What drugs cause decreased renal excretion?

A

Aspirin.
Thiazide diuretics.
Furosemide.
Ethambutol & pyrazinamide.

19
Q

What are the two types of drugs used for uric lowering therapy? Give examples.

A

Xanthine oxidase inhibitors:
> Inhibit uric acid synthesis.
> E.g. Allopurinol & Febuxostat.

Uricosuric agents:
> Increased the excretion of uric acid.
> E.g. Sulphinpyrazone, Probenecid, Benzbromarone.

20
Q

What drug shouldnt be used for ULT in patients with renal impairment? What should be used instead?

A

Allopurinol shouldnt be used.

Benzbromarone can be used in patients with renal impairment.

21
Q

What is pseudogout?

A

Arthritis caused by accumulation of calcium pyrophosphate dihydrate.

22
Q

What are the causes of pseudogout?

A
Hyperparathyroidism.
OA.
Diabetes.
Haemochromatosis.
Acromegaly.
23
Q

What demographic does pseudogout mainly affect?

A

Elderly women.

24
Q

What demographic does gout mainly affect?

A

Middle aged men.

25
Q

What joints does pseudogout mainly affect?

A

Knee’s & wrists.

26
Q

What is reactive arthritis?

A

Arthritis that develops following an infection elsewhere in the body.

27
Q

What is the most common cause of reactive arthritis?

A

Chlamidyia.

28
Q

What is Reiters syndrome?

A

Characteristic signs of reactive arthritis which include:
> Arthritis of large joints.
> Inflammation of the eye (i.e. uveitis).
> Urethritis or cervicitis.
> Psoriasis.

29
Q

What is sero-negative arthritis? Give some examples.

A

Arthritis with negative rheumatoid factor antibody in serum.
Examples:
> Reactive arthritis.
> Psoriatic arthritis - arthritis in individuals with psoriasis.
> Ankylosing spondylitis - chronic inflammatory arthritis of the vertebral column.
